Honeycomb-like appearance of dilated Virchow-Robin spaces.

چکیده
FIG. 1. — Case 1: Brain MRI FLAIR (A) and T2 (B) sequences showing bilateral honeycomb-like pattern involving both capsulo-lenticulostriatal areas and thalami corresponding to dilated VRS. Case 2: Brain MRI T1 (C) and T2 (D) sequences showing left hemispheric fronto-parietal parasagital dilated VRS. Virchow-Robin spaces (VRS) are interstitial fluidfilled pia-lined extensions of the subarachnoid space surrounding the arteries entering brain parenchyma. On MRI, dilated VRS have signal intensities identical to those of cerebrospinal fluid and are either a coincidental finding or found in patients with miscellaneous clinical conditions. (Kwee and Kwee, 2007). We present two cases with dilated VRS in different locations in whom vascular, infectious or metabolic disorders were ruled out. The first patient, a 50-year-old lady with long-standing schizophrenia, was admitted in Doha following a generalized tonicclonic seizure. Neurological examination was normal. MRI brain scans are shown in figure 1 A and B. The second 78 yo female patient consulted in Liège for gait instability. Her neurological examination was consistent with mild gait apraxia and associated with mild parkinsonian signs. L-Dopa reduced the latter, but not the former. MRI scans are shown in figure C and D. Dilated VRS can be found in three characteristic locations (Marnet et al., 2007): VRS surrounding lenticulostriate vessels in the anterior perforated substance prior to their entry into the basal ganglia (Type I) (case 1); along the perforating medullary arteries in the high-convexity white matter of elderly subjects (Type II); in the midbrain (Type III). Occasionally, dilated VRS may become very large and involve one hemisphere (case 2), show bizarre configurations and even cause mass effect (Papayanis et al., 2003). The mechanisms underlying dilated VRS as well as their clinical significance remain however elusive in most cases. MRI signal intensities and locations allow to distinguish them from other lesions like lacunar infarcts, demyelination, cystic periventricular leukomalacia, crypto coccosis, mucopolysaccharidoses, cystic neoplasms, neurocysticercosis, and various cystic lesions (Heier et al., 1989). Magnetic resonance imaging features of unusually dilated Virchow-Robin spaces--two case reports.
Two patients presented with unusually dilated Virchow-Robin spaces appearing as cystic lesions of varying size with signal intensity identical to the cerebrospinal fluid on all magnetic resonance pulse sequences. However, fluid-attenuated inversion-recovery (FLAIR) images disclosed small, high intensity foci adjacent to these cystic lesions in one patient. These high intensity foci on FLAIR ima...
متن کاملRating method for dilated Virchow-Robin spaces on magnetic resonance imaging.
BACKGROUND AND PURPOSE Dilated Virchow-Robin spaces are an emerging neuroimaging biomarker, but their assessment on MRI needs standardization. METHODS We developed a rating method for dilated Virchow-Robin spaces in 4 brain regions (centrum semiovale, basal ganglia, hippocampus, and mesencephalon) and tested its reliability in a total of 125 MRI scans from 2 population-based studies. Six inve...
متن کاملBrain MR: pathologic correlation with gross and histopathology. 1. Lacunar infarction and Virchow-Robin spaces.
MR imaging was performed on 36 formalin-fixed brain specimens. For three of these specimens, in vivo MR studies had also been performed before death. Changes that take place in the MR appearance of the brain after fixation are discussed. Gross and microscopic pathology revealed 14 lacunar infarctions in seven cases and enlarged Virchow-Robin spaces (état criblé) in four. Both types of lesion we...
متن کاملIs the Severity of Dilated Virchow-Robin Spaces Associated with Cognitive Dysfunction?
Virchow-Robin spaces (VRS) are perivascular spaces surrounding the perforating cerebral arteries or arterioles.1,2 They distribute throughout the subarachnoid space to the brain parenchyma. Solutes from the brain parenchyma and lymphatics are drained into VRS, but VRS themselves do not have direct communication with the subarachnoid space.3 Dilated VRS (dVRS) are easily detectable by magnetic r...
متن کاملMR appearance of Virchow-Robin spaces along lenticulostriate arteries: spin-echo and two-dimensional fast low-angle shot imaging.
PURPOSE To delineate the appearance of normal Virchow-Robin spaces on routine spin-echo images and demonstrate the lenticulostriate arteries within them with a flow-sensitive fast low-angle shot (FLASH) pulse sequence. METHODS Seventy subjects, free of cerebrovascular diseases and other neurologic disorders, ranging in age from 1 to 75 years, were examined. On a 1.5-T MR system, axial spin-ec...
